Dr. J. Wesley Cook is a Family Medicine Specialist and Addictions Medicine Specialist with more than 30 years of experience in the medical field. Dr. Cook’s mission has always been to provide quality health care to his patients while treating everyone with compassion and respect. He is an advocate for the LGBTQIA+ community, and passionate about serving patients of all genders, race, economic background, religion, and diagnosis.
Dr. Cook was born in St Louis, MO, and the oldest of 5 brothers. He came to Chicago for medical school and fell in love with the city of neighborhoods. In addition to his work with Thresholds Health, he also serves as the Chief Medical Officer at Thresholds, one of the oldest and largest community mental health and substance use treatment agencies in Illinois.
