We’re here to provide primary care services to the entire community, no matter if you have insurance or not. Walk in for an appointment today, our providers are accepting patients.
Children’s Health – Primary care
Thresholds Health provides well child services for children and adolescents, including:
- Well child visits
- Sick child visits
- Counseling for nutrition and physical activity
- Weight assessment
- Depression screen for patients 12 and older
- School and sports physicals
Adult Health – Primary care
Thresholds Health provides comprehensive primary care, disease prevention and management for adults, including:
- Physical examinations and well visits
- Vaccinations
- Chronic disease management (e.g. hypertension, asthma, diabetes)
- HIV screenings and linkage to care
- Depression screenings
- Colorectal screenings
- Statin theraphy
- Referrals for optometry
- Additional enabling/supportive services
- Pharmaceutical services: For uninsured patients, we have a structured sample distribution program and support our patients to apply for pharmacy discount programs. We maintain a limited supply of in-stock pharmaceuticals for urgent patient care needs administered on site.
